The agreement requires Hy‑Vee to provide de‑identified, aggregate lab data to Dallas County in accordance with HIPAA de‑identification standards (45 C.F.R. §164.514).
The contract sets per‑participant rates (for example, biometric and A1c screenings are listed at $45/participant in the contract text) and requires Hy‑Vee to provide licensed dietitians or pharmacists for events. It also includes a 90‑day termination clause, confidentiality and data‑security provisions and a statement that participants must sign consent forms before testing.
