Choptank Community Health honored as county proclaims National Health Center Week

Choptank Community Health leaders updated the Caroline County Board of Commissioners on the nonprofit health center’s local services and impact and the board issued a proclamation recognizing National Health Center Week.

Sarah Rich, president of Choptank Community Health, told the commission that “in 2025, nearly 13,300 Caroline County residents were served by Choptank Community Health,” representing roughly four in 10 county residents. Rich described primary care, pediatric and adult dental services, integrated behavioral health, prenatal and women’s health care, and school-based health centers and said the organization operates mobile units to reach residents in Goldsboro, Denton and Federalsburg.

The board read a proclamation recognizing the week in August 2026, noting Choptank’s multi-county service footprint and its role as a federally qualified health center that reduces barriers to care. President Jay Travis Breeding and Vice President Larry C. Porter were listed on the proclamation as signatories. Commissioners and staff praised Choptank’s local partnerships — including a pediatric dental residency partnership Rich said involves NYU Langone — and thanked the health center for its role in expanding access to care in a county without a hospital.

Rich left takeaway data with the commission for staff review and outreach; commissioners and Choptank agreed the recognition highlights ongoing collaboration between the county, schools and regional health providers.
