The Big Free Collaborative used a two‑stage straw poll on July 22 to narrow 28 nominated topics for 2027 down to six for further development. Chair Emily Trancy told members the goal is impact and implementability, and that each member should consider who will use the final guideline and whether implementation partners exist.
Members first completed an informal three‑choice poll to gauge interest and then voted again to pick six topics for development. Trancy announced the top six: AI governance in health care; malnutrition and nutritional risk (including concerns tied to GLP‑1 drugs); a revision of addiction and dependence treatment guidelines; opportunistic salpingectomy for ovarian cancer prevention; mobile integrated health; and pediatric pain management. "We will propose to move forward with these top 6 recognizing that they are only a little bit ahead of the next 2," Trancy said when announcing the results.
The process included public comment from clinicians, researchers and state staff who advocated for specific nominations. Chuck Drescher, a gynecologic oncologist, urged support for opportunistic salpingectomy and cited large‑cohort evidence of reduced ovarian cancer risk. The Collaborative will develop these six topics further and revisit others in future cycles; members were reminded that participation will require time and follow‑through from those who back a chosen topic.
