Appropriations advances pathway-to-licensure bill for internationally trained physicians, delays implementation for rulemaking

The Senate Appropriations Committee advanced S42 (also referred to in the hearing as S142), a bill establishing a staged licensure pathway for internationally trained physicians and medical graduates.

Under the bill as amended by the Senate Health & Welfare Committee, the pathway begins with a provisional license allowing practice at a participating health-care facility under physician supervision, followed by a limited license and eventual full licensure if milestones are met. The amendment delayed rulemaking to begin July 2027 and set the pathway to take effect July 1, 2028; the Department of Health and the Board of Medical Practice must deliver a report by January 2027 exploring alternative licensing pathways used by other states and outlining resource and statutory needs.

Joint Fiscal Office staff cautioned that starting the program will have costs — for example, other states hired consultants and medical boards often need additional staff — and that exact amounts will depend on program design and the number of participants. "By passing the bill, you're sort of committing to a future appropriation," a JFO analyst told the committee, noting that no appropriation is required now because implementation dates were pushed out.

Committee members said they expected additional detail about implementation and costs in the January 2027 report before final budget decisions. The committee moved the bill to the floor with the Health & Welfare amendment incorporated; the timeline for rules and program launch remains part of the bill text.
