Carmen, the community mentor administrator, reported that a recent complaint survey originating from an anonymous staff complaint listed an address in the central neighborhood (2901 Forbes). State surveyors initially went to the central address and later came to the West (Lehi) building; Carmen said the state acknowledged the initial routing error but ultimately inspected the West building.
The complaint appeared to be about a staff member’s demeanor toward coworkers. Carmen said the surveyor reviewed documentation showing the facility had already addressed prior concerns about that staff member and did not issue citations. She said the facility pushed back with state survey leadership because having three nursing homes in close physical proximity can complicate complaint routing and risk penalizing one facility for issues tied to a neighboring address. "We should be treated like three separate entities since we pay the licensure fee for all three," Carmen said.
Committee members discussed that complaint prioritization and timing can vary; Carmen noted state surveyors prioritize emergent allegations and that some non‑urgent complaints may be investigated months after they are filed. She said she will continue discussions with state survey leadership to seek clearer, consistent handling when multiple facilities are nearby.
