Senate advances and passes pilot program to create licensure pathway for international medical graduates

The Washington State Senate advanced and passed Substitute Senate Bill 5,185 on Feb. 5, 2026, establishing a pilot program intended to create a pathway to physician licensure for international medical graduates.

Senator Saldana urged support of the substitute bill, thanking international medical graduates, the Washington State Medical Commission and the Washington State Medical Association for shaping the proposal. She said the pilot would allow internationally trained physicians to work under supervision, expand family and primary care access in underserved parts of the state, and provide a pathway to permanent licensure.

Senator Mazzall emphasized that the medical commission and association had negotiated terms to ensure patient safety, saying professional oversight is intended to ensure that “those that are practicing medicine are doing it appropriately and safely.” After floor remarks and a roll call, the substitute bill was declared passed by constitutional majority.

Supporters framed the bill as a response to access gaps in parts of the state where appointment waits are long, arguing the pilot would enable qualified international graduates to continue providing care and eventually establish independent practices. The bill was referred to the human services committee during earlier process notes and moved through second and third reading on the floor before final passage.

The Senate’s action sends the measure forward for the remaining steps toward enactment.
