Public‑health partners, CHIP and a social‑prescribing pilot highlighted in Larimer County update

Public‑health staff told commissioners Larimer County has completed two years of its five‑year Community Health Improvement Plan (CHIP) and will host a CHIP event Feb. 24 (Drake Center, 8–10:30 a.m.) to launch social‑prescribing work and update resource connections.

Jo Buckley (Department of Health and Environment) said social prescribing will connect health‑care providers to local nonprofit supports and cited UCHealth’s use of the Find Help system as an example that needs county‑specific updates. Buckley also noted UCHealth’s community‑needs assessment has moved to implementation and that UCHealth will hold a public kickoff in April (Embassy Suites, Loveland, 2–3 p.m.).

On criminal‑justice‑related interventions, Emily Humphrey (Community Justice Alternatives) described the Sequential Intercept Model follow‑up: a core team will meet March 13 to prioritize 9–10 actionable items into roughly four topic areas and assign working groups to carry the work forward.

Buckley and Humphrey said the county will coordinate health partners, UCHealth and community nonprofits to implement these efforts. No formal board actions were taken during the presentation.
