Commission approves series of renewals and contract changes to fund intergenerational, nutrition, volunteer and CLF programs

San Francisco's Disability and Aging Services Commission on April 7 approved a set of grant renewals, contract modifications and new service agreements intended to sustain nutrition, intergenerational, volunteer visitor and Community Living Fund programs as the city moves through pandemic recovery.

- Episcopal Community Services congregate meal modification to support dinner program at the Granada Hotel/site conversion: $100,873 additional funding (modification for remainder of FY20-21) to establish a daily congregate dinner program for residents.

- Volunteer Visitor program renewals (Covia and Lighthouse for the Blind): $178,547 (plus contingency; not to exceed $196,431) to continue social-matching visitor services that reduce isolation.

- On Lok Day Services new grants for health promotion, chronic disease self-management and fall-prevention classes covering FY21-25: $2,308,732 (plus 10% contingency; total not to exceed $2,539,605). The programs include evidence-based diabetes and chronic-disease education and a USF-developed fall prevention course.

- Community Living Fund modification with Institute on Aging extending and funding CLF services through 06/30/2023: $9,788,758 (plus 10% contingency; total not to exceed $22,408,937 as presented). CLF provides coordinated case management, purchase-of-service gaps (rental subsidies, home modifications, home care) and targets individuals at risk of institutional placement.

Presenters noted program monitoring occurred in 2020 and found compliance with no corrective actions for the referenced grantees. Commissioners asked for demographic breakouts (including API and younger adults with disabilities) and placement/outcome metrics; presenters committed to report those data back to the commission. Where available, staff committed to circulate slides and appendices.

Why this matters: the approvals maintain local program capacity for home- and community-based services, nutrition, social connection and veteran supports at a time of high demand and evolving service delivery models. The CLF modification in particular represents a multi-year commitment to case management and purchase-of-service supports to avoid unnecessary institutionalization.
