The San Francisco Department of Disability and Aging Services (DOS) Commission on June 28 approved a series of grant agreements and contract modifications covering community centers, neighborhood-based programs, nutrition outreach, care navigation, and enhanced care management.
The largest item, a multi‑year authorization to contract with multiple providers to operate 41 community centers citywide, totals $47,954,320 with a 10% contingency (not to exceed $53,501,000). The package includes baseline service expectations for sites, co‑located services such as Aging and Disability Resource Centers, and a five‑site digital navigator pilot to help older adults and adults with disabilities bridge the digital divide.
Other approved grants and contract actions included:
- Community Music Center neighborhood choirs (Item 12A): $1,059,204 plus 10% contingency (total not to exceed $1,165,124); approved by roll call.
- HomeSafe (Institute on Aging) (12B): $5,000,000 plus 10% contingency (not to exceed $5,500,000); program supports eviction prevention and housing transitions for older adults and adults with disabilities.
- Neighborhood-based programs with multiple providers (12C): $2,913,264 plus 10% contingency (not to exceed $3,204,590).
- Shanti Project care navigation and peer-support grants for LGBTQ+ older adults and adults with disabilities (12D): $9,111,840 plus 10% contingency (not to exceed $10,023,024).
- Modification for Smart Money financial coaching with the San Francisco LGBT Community Center (12E): additional $78,092, revised total not to exceed $408,702.
- Intergenerational programming grants (12F): $3,596,760 plus 10% contingency (not to exceed $3,956,436).
- California Department of Aging MIPA contract and modification to Self Help for the Elderly HICAP (12H): additional allocation for Medicare outreach and enrollment assistance.
- CalFresh expansion with San Francisco Marin Food Bank (12I): $288,783 plus 10% contingency (not to exceed $317,661), targeting ZIP codes 94110, 94116 and 94121.
- CalFresh Healthy Living (SNAP‑Ed) with Self Help for the Elderly (12J): $412,418 plus 10% contingency (not to exceed $453,660).
- Food support for ZIP 94112 with Southwest Community Corporation (12K): $230,770 plus 10% contingency (not to exceed $253,847).
- Volunteer Visitor Program grants to Lighthouse for the Blind and Little Brothers Friends of the Elderly (12L): $400,000 plus 10% contingency (not to exceed $440,000).
- Enhanced Care Management contract with San Francisco Health Plan (12M): contract to provide ECM services for Community Living Fund clients who are SF Health Plan members under CalAIM; DOS will bill for covered services and use funds to offset CLF costs.
- Case management, clinical collaborative services, and Veterans Justice Case Management grants (12N): $17,427,704 plus 10% contingency (not to exceed $19,170,476).
Most items were approved by roll-call votes. The meeting transcript records the typical roll call as: Vice President Nelson Lum, Commissioner Martha Knudson, Commissioner Sasha Bittner, Commissioner Linda Parker Pennington, and Commissioner Wanda Zhang. For one large community‑center item a commissioner recused herself; the motion passed with the remaining commissioners voting in favor.
