Laguna Honda is meeting most settlement milestones, DPH says; CMS surveyors expected soon

San Francisco Department of Public Health officials told the Health Commission on March 7 that Laguna Honda Hospital is progressing under its settlement action plan toward recertification.

Director Dr. Grant Colfax said the program has an action plan with milestone requirements tied to the settlement agreement. For January and February the team documented 259 milestones and staff reported they were on track to have met them; March contains 77 additional milestones. Colfax said an independent quality improvement expert is confirming milestone completion and that CMS surveyors are expected to visit on an irregular cadence and do not provide advance notice.

Commissioner questions focused on next steps in the recertification process and the timing and nature of expected surveys. Dr. Colfax and staff said they are preparing for surveyors and maintaining the quality‑improvement work needed to demonstrate compliance and readiness for recertification.

The commission did not take a vote on Laguna Honda at this meeting; the update was informational and designed to reassure commissioners the facility is addressing items required under its agreement and preparing for imminent surveys.
