Laguna Honda Hospital discharges six residents after care level reassessment

Significant progress in resident care at Laguna Honda was highlighted during a recent San Francisco government meeting, with officials reporting that 39 residents have transitioned away from needing skilled nursing care. This marks a notable improvement since early April, when 42 residents were identified as eligible for reassessment. Of the six residents discharged recently, one returned home while five moved to board and care facilities, reflecting a successful shift in care levels.

The meeting also introduced a new initiative aimed at enhancing bedside care. Known as the Consistent Care at the Bedside Monitors (CCBMs), this program involves expert consultants embedded in each of the 13 nursing units. These seasoned professionals, many with decades of experience, will work closely with nursing staff to improve training, accountability, and overall care quality. The initiative is expected to last through December and aims to foster lasting cultural changes within Laguna Honda.

In terms of regulatory compliance, the latest monitoring survey conducted by the Centers for Medicare & Medicaid Services (CMS) revealed a significant reduction in regulatory findings, dropping from over 120 to just 44. This decline is seen as validation of the improvements made at Laguna Honda, bolstering the facility's position as it prepares for recertification. Officials expressed optimism about submitting the recertification application this summer, pending the formal findings from the latest survey.

As the meeting concluded, public comments reflected ongoing concerns about patient care, with one speaker poignantly questioning the lack of transparency regarding patient deaths. This underscores the critical need for continued dialogue and accountability as Laguna Honda moves forward in its recovery and improvement efforts.
