County staff described how an encampment "by-name" consult group and contracted case management are being used to coordinate individualized pathways out of unsheltered homelessness.
"There was 235 residents have been identified on the encampment list since inception. Currently, there's 204 residents active on the list. 20 of those residents have... been placed in housing," Wilkins said, adding that those placements reflect work by the broader Community Wellness Response (CWR), city partners and HSD rather than HSD alone.
Staff gave a breakdown of coordinated-entry assessment statuses for the encampment list: 117 people had completed coordinated-entry assessments, 12 had not completed one, 20 needed updated assessments and 6 were not eligible because they did not meet the chronic/long-term definition used for single adults.
Wilkins and other staff described outreach cadence changes after the Pig's Eye encampment closure: staff moved from daily outreach to targeted twice-weekly visits at Randolph and periodic visits at smaller sites to preserve established trust and to free capacity for back-office casework and coordinated-entry referrals.
